​Closure Notice

Dear clients, families, and professional colleagues,
 
The office of FASDx Services LLC, the referral hub agency for the Anchorage FASD Diagnostic Team, will close permanently on January 31, 2020.  At this time there is no clear plan for private sector, non-Native clients  to obtain an FASD diagnosis in Anchorage.
 
We feel so fortunate to have learned so much from our clients and our team colleagues over the past 10 years as we worked to provide a quality, comprehensive evaluation for those who sought our services. Stigma continues to make seeking an FASD diagnosis a challenge but we are hopeful that a 'phoenix can rise from the ashes' as this FASD team support office closes. 
 
We will be securely keeping a copy of the FASD Team Diagnosis Summary record for 7 years beyond the time of the assessment for those who were 18 or over at the time of the assessment
OR
Up until age 21 for children who were evaluated under the age of 18
 
We have attempted to contact all prior clients by email and social media posts regarding our office closure but if we missed you and you need a copy of your FASD evaluation, please contact Marilyn Pierce-Bulger, ARPN at marilynpb@fasdxservices.com   
 
No Release of Information (ROI) is necessary to request your own report, or if you are the parent of a minor child or guardian of an individual. However, identifying information (such as client date of birth & proof of your relationship to them) will need to be verified.
 
All other record requests must include a completed ROI signed by the appropriate person. Requests without appropriate signatures cannot be completed.
 
Evaluation reports completed by the Anchorage FASD Diagnostic Team at Assets, Inc. (2009-2014) or at FASDx Services, LLC (2014-2019) are available by request, as described above.
